Registration
Advance registration is encouraged, as space and materials are limited.
To qualify for the discounted registration fee, the completed form
and payment must be received by the Cancer Center on or before September
8, 2008. The registration fee includes conference tuition,
materials, four breakfasts, breaks and the 10th Anniversary Reception.

The Thursday mini-symposia are included in your
registration fee.
Letter of Invitation for VISA Policy
An official Letter of Invitation to obtain a VISA and authorization
to attend the 10th Annual Lynn Sage Breast Cancer Symposium will
be issued ONLY after the Registration Form and full payment have
been received.
Refund/Cancellation Policy
Registration fees less a $50 administrative handling fee are refundable
if a written request is received on or before September 22, 2008.
Please include your registration ID number or Social Security number
when requesting a refund. No refunds will be granted after September
22, 2008, however you may transfer your registration to a colleague.

Conference Location and Accommodations The Fairmont Chicago is conveniently located in the heart
of Downtown Chicago and at the south end of Chicago’s “Magnificent
Mile” overlooking the fabulous Grant and Millennium Parks.
A limited number of guest rooms have been held for the Symposium
at the discounted rate of $265 plus tax per night through October

Ground Transportation
Chicago is served by two airports, O’Hare International and
Midway International. Taxi cabs are available at an estimated cost
of $45 from O’Hare and $35 from Midway. In addition, there
is an Airport Express shuttle that runs directly to the Fairmont
Chicago from both airports. Arrangements for the shuttle can be
made in the Baggage Claim area at either
airport or in advance at www.airportexpress.com.
Faculty Disclosure
Current guidelines state that participants in CME activities should
be made aware of any affiliation of financial interest that may
affect the speaker’s presentation. Each speaker
has been requested to complete a faculty disclosure form. The names
of faculty members declaring a relationship or who will be discussing
off–label usage will be indicated in the course syllabus.
