2011 Registration Information

Accreditation Statement
The Northwestern University Feinberg School of Medicine is accredited by the Accreditation Council for Continuing Medical Education to provide continuing medical education for physicians.

Credit Hour Statement
The Northwestern University Feinberg School of Medicine designates this live activity for a maximum of 23 AMA PRA Category 1 Credit(s)™. Physicians should claim only the credit commensurate with the extent of their participation in the activity.

Participation at the Lynn Sage Breast Cancer Symposium will fulfill up to 16 hrs of the required breast specific AMA/PRA Category 1 CME credits for the Mastery Program of the American Society of Breast Surgeons.

Registration
Advance registration is encouraged, as space is limited. To qualify for the discounted registration fee, the completed form and payment must be received by the Lurie Cancer Center on or before August 17, 2011. The registration fee includes conference tuition, materials, four breakfasts, breaks and the evening reception.

Letter of Invitation for VISA Policy
An official Letter of Invitation to obtain a VISA and authorization to attend the 13th Annual Lynn Sage Breast Cancer Symposium will be issued ONLY after the Registration Form and full payment have been received.

Refund/Cancellation Policy
Registration fees less a $50 administrative handling fee are refundable if a written request is received on or before August 15, 2011. Please include your registration ID number and Social Security number when requesting a refund. No refunds will be granted after August 15, 2011, however, you may transfer your registration to a colleague. For further information call Denise Marshall at 312.695.1392.

Faculty Disclosure
Current guidelines state that participants in CME activities should be made aware of any affiliation of financial interest that may affect the speaker’s presentation. Each speaker has been requested to complete a faculty disclosure form. The names of faculty members declaring a relationship or who will be discussing off–label usage will be indicated in the course syllabus. All faculty will be disclosing.
